Posterosuperior irreparable rotator cuff tears (PSIRCTs) are the shoulder equivalent of showing up to trauma call and realizing anesthesia is short-staffed, the implant tray is incomplete, and the patient is 64 years old with expectations of a 28-year-old CrossFitter.
These are tough shoulders — painful, weak, externally rotated into despair — and not yet arthritic enough to justify a reverse. So, what’s a shoulder surgeon to do?
